Summary (English)
This is the appendix to the report: "The Likelihood of Enrolment: Evidence from Haryana." It contains the questionnaire used for a survey conducted among the households of the Indian state of Haryana concerning the enrolment to "Rashtriya Swasthya Bima Yojna" (RSBY), a voluntary, Government sponsored health insurance scheme for poor households started in April 2008. The aim of this survey was to analyze how socio-economic and demographic factors influence the enrolment to RSBY.

Il contient le questionnaire utilisé pour réaliser un sondage parmi le familles de l'Etat Indien de l'Haryana et concernant l'inscription au "Rashtriya Swasthya Bima Yojna" (RSBY), un système d'assurance maladie voluntaire financé par le gouvernement indien qui s'adresse aux familles démunies et qui est opérationnelle depuis avril 2008. Le but de ce sondage était d'analyser l'influence des facteurs socio-économiques et démographiques sur l'inscription au RSBY.
