Income security

Target: in principle, all formal sector workers.
Benefits: according to Labour Law No. 13/2003, employers are obliged to pay full salary to their employees in case they are absent because of sickness. Employees cannot be terminated because of sickness unless they are absent for at least 12 months. Female employees should be given three months of paid leave during childbirth. Civil servants are entitled to similar provisions.

* Askesos - Sickness grant tor informal economy workers

Target: all informal economy workers.
Benefits: in case of sickness or injury, workers receive IDR 300,000 (maximum one claim per person and per benefit per year).

A new initiative is being developed to transform the Askesos programme to be more in line with insurance principles, as mandated by regulations on social security. The initiative, currently in trial phase, will be conducted in partnership with PT Jamsostek.

Delivery care, maternal and neonatal health

* Jampersal - Universal delivery care | data

Target: Women.
Benefits: Jampersal is a new Ministry of Health programme (started early 2011) that provides women universal free delivery care, including pre-natal and post-natal consultations. The scheme uses a direct payment mechansim based on a flat rate capitation amount, which means the patients do not pay anything. The total delivery package cost is IDR 420,000, including IDR 350,000 for delivery, IDR 40,000 for four anti-natal care visits, and IDR 30,000 for three post-natal care visits.

* Program Keluarga Haparan (PKH) - Maternal and neonatal health for the informal sector

Target: the conditional cash transfer PKH programme is primarily designed to improve maternal and neonatal health as well as children's education among poor households. Beneficiaries consist of households with children younger than 15 years of age (also includes children 15-18 years who have not yet completed the ninth grade) and/or pregnant or lactating women. In 2012, PKH covers 33 provinces and 1.5 million very poor households. The programme is expected to reach 3 million households in all districts by 2014 (Indonesian State budget 2012; Ministry of Social Affairs, 2010; consultation with staff of the Ministry of Social Affairs in 2011).
Benefits: Depending on the family structure and their compliance with the programme's educational and health requirements, households receive IDR 600,000 to 2,200,000 per year. Programmed conditions include: (1) children are enrolled in school and attend at least 85% of school days; (2) pregnant and lactating mothers as well as infants of 0-6 years of age regularly visit health facilities for health checks.

Regulatory Framework (ISSA)

First law: 1957 (Decree of the Minister of Labor No. 15).

Current law: 1992 (Act No. 3, employees' social security).

Type of program: Social insurance system. Medical benefits only.

Coverage

Employees of establishments with 10 or more employees or a monthly payroll of at least 1 million rupiah.

Exclusions: Employees whose employer provides benefits that are more comprehensive than those provided by the Jamsostek program, and employees with labor contracts of less than 3 months.

Voluntary coverage for self-employed persons.

Coverage is being extended to employees of smaller establishments and to organized informal-sector workers, including family labor, fishermen, and employees of rural cooperatives.

Special system for civil servants, civil service pensioners, military and police pensioners, veterans, national independence pioneers, and for their respective dependents up to age 25.

Source of funds

Insured person: None.

Self-employed person: 3% of monthly declared earnings for single employees; 6% for married employees.

Employer: 3% of monthly payroll for single employees; 6% for married employees.

The maximum monthly earnings used to calculate contributions are 1 million rupiah.

Government: None.

Qualifying conditions

Cash sickness and maternity benefits: No benefits are provided.

Medical benefits: Must be currently covered.

Cash benefits for insured workers

Sickness benefit: No benefits are provided.

Maternity benefit: No benefits are provided.

Medical benefits for insured workers

Medical benefits: Medical benefits include primary and specialist outpatient care, hospitalization, medicine, and emergency, dental and eye care. Eyeglasses, hearing aids, and prostheses are reimbursed, up to a maximum.

Insured persons must register with a primary care provider who is under contract with Jamsostek. A doctor's referral is required for access to specialist and inpatient care (except for emergencies).

The cost of maternity care for up to three children, up to 500,000 rupiah each.

Inpatient care is provided for up to 60 days a year.

Specialist care is provided.
